The Nurse Family Partnership

 

Nurse-Family Partnership is a voluntary program where highly educated nurses visit low-income women in their homes during their first pregnancy and throughout the first two years of their children's lives to accomplish three goals: Improve pregnancy outcomes by helping women improve their health related behaviors; Improve child health and development by helping parents provide more responsible and competent care for their children; and Improve families' economic self-sufficiency by helping parents develop a vision for their own future, plan future pregnancies, continue their education and find work. It is an intensive home visitation program for teens and young women under 24 years old, who meet the eligibility requirements. A public health nurse will visit the first-time mother in her home or other location throughout the pregnancy and until the child's second birthday. She will assist the new mother and her family by providing education and support in the areas of prenatal care, nutrition, childbirth, breastfeeding, child development, child safety, and setting and reaching personal goals.   Goals include:   fostering healthier pregnancies, improving the health and development of children and encouraging self-sufficiency.  

Program Eligibility:

  • Under 24 years old
  • Less than 28 weeks pregnant*
  • First time pregnant and parenting
  • Eligible for Medi-Cal, CalOptima or emergency/pregnancy related Medi-Cal
  • Desire to be involved in this program
