Bridges for Newborns

 

The Bridges for Newborns program is a centerpiece of the Children and Family Commission's Healthy Children initiative Initiative . The Commission partners with a network of birthing hospitals and a network of service providers to identify children whose families could benefit from additional family support and link them to a network of those services. The program assists parents in enrolling their children in health insurance programs, and in establishing relationships with clinics and doctors for immunizations and well-child checkups.

If a family is identified to be a good fit for Bridges for Newborns Home Visitation services, they will be offered services for up to a year or more, with an average of ten home visits per year, per family. Each client is unique and will receive a different level of care, based on need. Bridges case management services include health and developmental assessments by a multidisciplinary team, parent education, breastfeeding education and support, health safety screening and injury prevention education as well as linkages to community resources. Services are limited to those clients referred by partnering "Bridges" hospitals only.

Bridges for Medically High Risk Newborns

The Medically High Risk Newborns Program (MHRN) provides Public Health Nursing home visitation services to medically fragile infants until they reach 24 months of age. Nursing interventions include case management, parent education, counseling services to children and families who have medical and psychosocial risk factors which may impede the child's access to health services , and family self-sufficiency.

Services Include:   Physical, developmental and nutritional assessments,  Safety assessments,   Parent/child interactions,   Problem identification, Health teaching,   Case management and follow-up , and Referrals to community resources. Enrollment criteria include being a graduate of a Neonatal Intensive Care Unit   (NICU),   born at or discharged from a " Bridges Hospital ", and enrolled within 90 days of hospital discharge, and at risk for developmental delays based on one or more of the medical eligibility criteria.
