The Orangewood Children's Foundation and FaCT seeks to strengthen individual programs that promote healthy communities, strong families and reduce child abuse in Orange County.   OCF provides technical assistance and administrative oversight to a number of Prop 10 home visitation programs and services through training, technical assistance and program planning efforts.  

These Home Visitation programs services are carried out by community based agencies and the Orange County Health Care Agency - offering in-home nursing, parent education and mental health services for pregnant women and families with children 0-5.   Each program serves a unique aspect of prenatal and postnatal family support; all seek to promote optimal health outcomes and strong families for Orange County's infants and children.

Orange County Introduces the Child Health Diary

The aim of the Child Health Diary is to assist parents with the attainment and collection of vital health information that can be shared amongst providers and tracked for preventive purposes.   It is hoped that this tool will also assist to inform physicians and other providers and/or specialists that are working with and serving their patients, as well as know about other efforts that are in place to educate their families about pertinent anticipatory guidance/child development topics.

Developed by the Children and Families Commission funded Bridges Network, the goal of this tool is to improve communication and care between non-profit health programs funded by the Children and Families Commission of Orange County and pediatricians. The diary was designed to include useful information for parents, physicians, and other health/social service providers.   The diary can provide a "snap-shot view" of what topics have already been discussed with the family, hopefully providing an opportunity for the provider to focus more of their time on necessary gaps of information.  

The size of an immunization card and placed within a clear plastic sleeve, the diaries are being distributed and used by newborn home visiting programs, Family Resource Centers, Orange County Public Health Field Nursing, community based clinics, and other non-profit agencies.
