The Bridges Maternal Child Health Network (MCHN), a multi-disciplinary system of providers that collaborate throughout Orange County to improve the health and well-being of pregnant women and young children 0 – 5 years of age. Under the management of the Orangewood Children’s Foundation (OCF) and funding from the Children and Families Commission of Orange County (CFCOC), Bridges MCHN represents a strong family-friendly infrastructure of birthing hospitals, Public Health Nurses (PHN), and non-profit home visiting providers.   These providers work together to deliver regional programs that address a continuum of needs and deliver an array of strength-based, culturally relevant services.

 

Bridges MCHN provides early outreach, referral and support services to families to enhance maternal child health in Orange County.
