The Bridges Maternal Child Health Network (MCHN), a multi-disciplinary system of providers that collaborate throughout Orange County to improve the health and well-being of pregnant women and young children 0 – 5 years of age. Under the management of the Orangewood Children’s Foundation (OCF) and funding from the Children and Families Commission of Orange County (CFCOC), Bridges MCHN represents a strong family-friendly infrastructure of birthing hospitals, Public Health Nurses (PHN), and non-profit home visiting providers.   These providers work together to deliver regional programs that address a continuum of needs and deliver an array of strength-based, culturally relevant services.

 

Bridges MCHN provides early outreach, referral and support services to families to enhance maternal child health in Orange County.

The following confidential services are available to families countywide in English, Spanish and Vietnamese through Bridges MCHN at no charge:

 

Early Outreach & Referral

Bridges MCHN Hospitals provide early outreach to promote healthy pregnancies through prenatal education and distribution of the Commission’s Kit for New Parents After delivery mothers identified for risk are encouraged to breastfeed, and receive parent education on topics such as immunizations and health insurance. Additionally they are screened for specialized need and offered referrals to community partners within and outside the Bridges MCHN. Ten hospitals throughout the county participate in the network with administration provided by the Hospital Association of Southern California.

Prenatal Home Visitation

MOMS Orange County provides home visits by trained staff during pregnancy and following birth, focusing on prenatal and post partum health and education, breastfeeding support, maternal and well-woman health screenings and healthy infant development. Mothers must be pregnant, reside in Orange County and have self-declared financial need for this program.

Perinatal Substance Abuse Initiative/Assessment and Coordination Team (PSASI/ACT) a program of the Health Care Agency provides Public Health Nursing visitation for substance abusing (or those who have had a history of drug or alcohol abuse in the past) and/or HIV infected pregnant and postpartum women (up to one year) and their families. Intensive case management services and home visitation help to ensure optimal pregnancy outcomes.

Nurse-Family Partnership® is an evidence-based program partnering with the Heath Care Agency Public Health Nursing to provide home visitation to first time moms.  The program provides support, education and case management throughout pregnancy until the baby is two years old with the goal of improving pregnancy outcomes; child health and development; and economic self sufficiency of the family.  Must be a first time mom less than 24 years of age; eligible for Medi-Cal or WIC; and less than 28 weeks pregnant.

Specialized Support Services

Bridges for Medically High-Risk Newborns (MHRN) is a program of the Health Care Agency Public Health Nursing to provide in-home services to medically fragile infants until they reach 36 months of age.  Families are referred to this program from the hospital with the goal to prevent or minimize developmental disabilities or delays; promote infant’s optimal health & development; provide nursing support to infants & their caregivers; and coordinate with other healthcare providers and community agencies.

Health Access Promotion Program provides short-term, home visitation services to promote health access and education to pregnant women and families with children through 3 years of age.  Promotoras and Public Health Nurses provide services to promote access to health insurance, medical home, immunizations, home safety, nutrition education, oral health, breastfeeding support, community resource linkages, prenatal education and support, health and developmental screenings for at-risk children.  This program is available at the La Habra FRC for the city of La Habra and South County FRC serving Costa Mesa and the South County.

Infant Home Visitation is a 6-month program serving families with children birth – 17 months of age utilizing PIPE (Partners in Parenting Education), KIPS (Keys to Interactive Parenting Scale), and the ASQ (Ages & Stages Questionnaire) for at-risk infants.  Parents that would benefit from the services of this home visitation program have:

  • Unrealistic expectations about child development / discipline concerns;
  • Insufficient bond or poor parent-child interactions;
  • Or are at-risk of forming poor attachments

Referrals for this program must come from professionals/agencies only.  No self-referrals.

Toddler Home Visitation is a 3-6-month program serving families with children 18 months – 5 years of age utilizing Triple P (Positive Parenting Program)®  for high-risk children to promote positive behavior; improved relationships between parents and children; hands-on parenting practice; effective age-appropriate communication strategies via modeling; and positive, consistent and decisive ways to manage problem behavior.  Primary issues addressed during services:

  • Aggressive behavior / frequent temper tantrums
  • Establishing positive behavior routines
  • Defiance / ADHA – type symptoms
  • Attachment / bonding
  • Increasing skills of parents to promote strong families
  • Promoting good sleeping habits.
