In collaboration with FaCT, PC.FRC Health Access works within the FRC platform to provide health support services and education for pregnant women and families with children ages 0-5 years. PC.FRC focuses on improving immunization status, health insurance enrollment, access to a medical home, health assessments, community health education, intensive home visitation, and case management services.
